Your Impact

As an Advanced Practice Provider, you’ll provide comprehensive, interdisciplinary primary care to a dedicated panel of patients. You’ll have the opportunity to care for patients across both routine and acute needs while partnering with the broader care team to improve outcomes.

Your responsibilities will include:

Providing comprehensive primary care through in-person and telehealth visits.

Managing routine, acute, and chronic medical conditions.

Completing comprehensive Annual Wellness Visits and Health Risk Assessments.

Working collaboratively with physicians and interdisciplinary team members to coordinate care and address the whole person’s health needs.

Using evidence-based guidelines and clinical best practices to deliver high-quality care.

Identifying opportunities to improve care delivery, patient outcomes, and the patient experience.

Using data and quality metrics to identify opportunities for improvement and achieve strong results across measures such as STARS, HEDIS, utilization, and patient experience.

Practicing with a continuous-improvement mindset and helping develop innovative solutions to real challenges facing our patients.

What You’ll Bring

We’re looking for a compassionate, collaborative Advanced Practice Clinician who is committed to delivering high-quality, patient-centered primary care.

You’ll ideally have:

2+ years of experience practicing in a primary care setting, such as Internal Medicine, Family Medicine, or Geriatrics.

Experience with HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).

Comfort working across multiple technology platforms, including electronic health records and spreadsheets.

Strong organizational and time-management skills, with the ability to effectively manage multiple priorities.

A continuous-improvement mindset and enthusiasm for using data to improve care.

Excellent communication and collaboration skills.

An active, non-probationary state medical license.

Authorization to work in the United States.
