The Program Director, reporting to the Executive Director, oversees critical PACE (Program of All-inclusive Care for the Elderly) functions that include, but are not limited to, nutrition, social work, day center, personal care services, scheduling, and participant experience.  In addition, the Program Director is accountable for developing and maintaining a high-functioning Interdisciplinary Team (IDT).

Essential Job Duties:

Manage critical PACE functions including, but not limited to, hiring, training, supervising, and mentoring staff, and ensuring clear and effective workflows

Develop, plan, and facilitate an effective IDT and other participant-care related meetings, ensuring that they are organized and engaging, and that follow-up items are documented and tracked

Effectively oversee staff within their reporting structure to drive quality care and ensure adherence to Standard Operating Procedures (SOPs) and key regulatory requirements

Collaborate to achieve high participant outcomes with shared participant services, including but not limited to, home care, home health, transportation, central call center support, and utilization management

Oversee the WelbeHealth program site, supplies and equipment management, which includes monitoring and coordinating all ongoing maintenance and vendor oversight

Job Requirements:

Bachelor's degree in a relevant field such as health admin, health care management, nursing administration, gerontology, social work, business administration; five (5) or more years of relevant management experience may be substituted; master’s degree preferred

Professional experience in a relevant field such as nursing, social work, psychology, or recreational/occupational/physical therapy, with demonstrated competence in working with frail elderly living at home required

Minimum of seven (7) years of experience, three (3) of which must be in a supervisory capacity, managing a senior health care program, outpatient clinic, or healthcare facility

Multi-disciplinary management experience (nursing, social work, rehab)

Value-based care preferred

Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Compensation Offering

$145,399.15—$191,926.88 USD
