Medical Director I
WelbeHealth PACE (Program of All-Inclusive Care for the Elderly) provides seniors with the opportunity to continue living in their homes and in their communities. We employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care. The Medical Director I performs comprehensive in-person assessments and coordinates with the IDT to develop an individualized care plan for each participant. At the direction of the Medical Director II, this role oversees all participant care in nursing homes and hospitals, including admits and discharges for acute, short-term, and long-term placement, and the use of medical specialists to ensure effective care coordination. The Medical Director I also provides direction and support to Nurse Practitioners and other members of the IDT.

Regular hours of 8am-5pm, Monday through Friday, including home visits

On the day-to-day, you will:
Provide assessments, planning, primary care, and continuous medical coverage to participants
Collaborate with community-based physicians and the IDT in discharge planning from acute or long-term settings
Actively participate in IDT meetings, communicating participant changes, collaborating on care planning decisions, and coordinating 24-hour care delivery
Help maintain participant care in nursing homes, making regular visits as dictated by participant need
Job requirements include:
Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education
Master’s degree or doctorate degree
Board certification – internal medicine, family practice, or emergency medicine; geriatrics preferred
Active State Physician License without restrictions, as well as state CDS certification and DEA registration
Must be willing and able to participate in rotating night and weekend call
