ABOUT THE ROLE
As a Licensed Practical Nurse (LPN) at Hopscotch Primary Care you’ll play a meaningful role in delivering compassionate, relationship-centered care to older adults in rural communities. We’re looking for someone who is clinically skilled, patient-focused, and excited to be part of a collaborative care team that truly values connection, advocacy, and whole-person care. In this role, you’ll build trusted relationships with patients while partnering closely with providers and teammates to create an exceptional care experience.

WHAT YOU’LL DO
In this role, you’ll help create a welcoming, supportive experience for patients while providing high-quality clinical care under the supervision of an RN, APP, and/or physician. Responsibilities include rooming patients, assisting with assessments and screenings, administering medications and vaccinations, supporting care coordination, performing diagnostic testing, and helping patients navigate both clinical and community resources. You’ll also work closely with the broader care team to improve outcomes, address social determinants of health, and ensure patients feel supported every step of the way.
ABOUT YOU
We’re looking for a compassionate and motivated LPN who enjoys building relationships and delivering patient-centered care. The ideal candidate is adaptable, collaborative, detail-oriented, and energized by working in a team environment where no two days are exactly alike.
Qualifications include:
Active North Carolina LPN license
BLS certification
1+ years of LPN experience (primary care experience strongly preferred)
EMR proficiency required
Strong communication, organization, and customer service skills
Positive attitude, accountability, and a willingness to jump in where needed
Valid driver’s license and ability to meet motor vehicle requirements
