Job Responsibilities:
Discuss and review patients' medical history, symptoms, allergies, and current medications
Perform comprehensive physical examinations and clinical assessments
Ask patients situation-specific questions to formulate accurate diagnoses
Diagnose and treat acute and chronic medical conditions
Develop and implement individualized treatment plans focused on improving patient outcomes
Prescribe suitable medications and provide proper dosage and administration instructions
Order, review, and interpret laboratory, imaging, and diagnostic test results
Provide preventive care services, wellness visits, health screenings, and patient education
Collaborate with care teams and specialists to coordinate comprehensive patient care
Manage patient panels and support value-based care initiatives, quality measures, and population health goals
Maintain accurate and timely documentation in eClinicalWorks (eCW)
Job Qualifications:
Doctor of Medicine (MD), Nurse Practitioner (NP), or Physician Assistant (PA)
Board-certified in Family Medicine, Internal Medicine, or a related field
Active Arizona license or ability to obtain prior to start date
Experience providing primary care services in an outpatient setting preferred
Sound medical knowledge
Experience utilizing electronic medical record systems, preferably eClinicalWorks (eCW)
Knowledge of value-based care models, quality measures, and population health management preferred
Excellent analytical and problem-solving skills
Exceptional communication skills
A patient and compassionate disposition
