Promote Throughput Reliability Work with post-acute partners to improve referral responsiveness, documentation alignment, authorization readiness, and discharge progression reliability.

Support Preferred Network Performance Standards Reinforce established performance expectations and tiering criteria through routine engagement and performance transparency.

Strengthen Clinical Alignment Conversations Collaborate with facility clinical leadership to review readmission diagnoses, high-impact DRG trends, and transition breakdowns that may impact system performance.

Maintain Structured Performance Documentation Track meeting outcomes, barrier logs, corrective actions, and performance improvement initiatives in accordance with departmental governance standards.

Provide Intelligence to Manager and Director Escalate systemic trends, recurring patterns, and high-variance performance issues to leadership to inform enterprise-level decision-making.

Support Network Expansion and Governance Scaling Assist with onboarding of new partner facilities into the governance model as the network expands across SNF, IRF, LTACH, and home-based settings.
