About the Role

This is a field-based sales role focused on growing our patient population.

As an Outreach Leader, you'll be responsible for identifying, developing, and managing relationships that generate new patients for Hopscotch Primary Care. You'll spend your time in the community meeting with prospective patients, local organizations, referral partners, businesses, senior groups, and community leaders to educate them about Hopscotch and help eligible individuals enroll as patients.

Success in this role comes from building trust, creating new opportunities, developing a strong sales pipeline, and consistently achieving patient growth goals.

You'll have significant autonomy to build your territory, execute your outreach strategy, and drive measurable results.

This position is home-based in the greater Marion, NC area and requires daily local travel.

What You'll Do

Drive Patient Growth

Own patient acquisition goals within your assigned territory

Build and execute a territory sales plan to achieve monthly and quarterly enrollment targets

Prospect for new opportunities through networking, cold outreach, community engagement, and referrals

Identify, qualify, and convert prospective patients into active Hopscotch patients

Develop and maintain a healthy pipeline of leads and manage opportunities through every stage of the enrollment process

Track sales activity, conversion metrics, and pipeline performance using CRM tools

Build Community Partnerships

Develop strong relationships with community organizations, senior centers, churches, employers, healthcare providers, pharmacies, and other referral sources

Identify new partnership opportunities that increase awareness and patient referrals

Serve as the face of Hopscotch Primary Care throughout your territory

Represent Hopscotch at community events, health fairs, educational programs, and networking opportunities

Relationship Management

Build lasting relationships with patients, caregivers, and community partners

Follow up with new patients to ensure a positive onboarding experience

Collect market intelligence and community feedback to improve outreach efforts

Collaborate closely with Marketing, Clinic Operations, and Care Teams to maximize growth opportunities

Sales Planning & Performance

Develop territory growth strategies and action plans

Monitor key sales metrics including leads, conversions, enrollments, and partnerships

Maintain accurate CRM documentation and activity reporting

Continuously evaluate outreach efforts and adjust strategies to improve results

Qualifications

Required

High school diploma or equivalent

2+ years of sales, business development, account management, or customer-facing experience

Valid driver's license and reliable transportation

Ability to travel locally throughout the assigned territory

U.S. work authorization

Preferred

Bachelor's degree

Healthcare, Medicare, insurance, or value-based care experience

Experience using a CRM (Salesforce or similar)

Experience working with older adults or community organizations
