Care Coordination Data Entry Specialist
Position Overview:
The Care Coordination Data Entry Specialist works independently assisting the Operations Teams with delegated tasks as needed such as enrollment side duties, outreach to members, mailing documents, and data entry into state systems. This position will be supporting members from different health plans and products.
Schedule: Full time position, day shift hours, no evenings, weekends or holidays. Hours are 8am to 5pm Monday thru Thursday & 8am to 3pm on Fridays.
Location: Include description of radius as necessary
Salary: $24.00 - $25.00 per hour. Salary will be commensurate with experience.
Responsibilities:
Assist with mailing out time sensitive documents to members
Complete outreach calls to members and or legal guardians to follow up on necessary documentation needed for support plans
Work efficiently and accurately in the MnChoices system
Looking up members in MNITs for eligibility of waivers and insurance information
Stay up to date with health plan changes and requirements to stay in compliance
Participate in regular team meetings
Participate in regular health plan meetings
Other duties as assigned by the Operations Manager
Qualifications:
Education/Certification/Experience
Associate’s degree in Human Services, Public Health Community Service, etc.- Preferred
Ideal candidate will have 3+ years experience working in a related field (hospital, HUC, customer service, CMA or other related field)
Intermediate-level computer proficiency with email, fax, word processing, spreadsheets, and data bases. Experience using Google Suite - Required
