Case Manager/Educator- Justice, Accountability and Recovery Program
About the Role:
The Case Manager (“CM”) is an integral member of Community Reach Center’s Clinical Teams (“Division”). The CM provides clinical and support services to consumer and/or their families through integrated behavioral healthcare services. The CM will utilize a Psychosocial Rehabilitation Model that emphasizes assisting persons to explore and develop social, occupational, leisure, and living skills. Provides treatment that supports consumers in living in the community successfully including linking consumers with resources to obtain and/or learn basic needs. Additionally, the CM will have other duties and responsibilities as determined from time to time by the Program Manager.
Essential Functions:
Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families
Provides outpatient restoration education services for consumers going through competency court proceedings.
Support and closely coordinate mental health and substance abuse care with the consumer’s clinical care team and the primary care provider
Attend collaborative treatment team meetings and reviews of consumers
Provides crisis de-escalation when needed
Assist eligible consumers in gaining access to needed medical, social, educational, and other services
Provide an assessment of needs and service plan development
Empowers consumers to reach their goals and sets a positive example and provides role modeling as well as technical expertise to accomplish goals
Provide transportation for consumers in agency vehicles when needed
Will work closely with outside organizations on resources and care coordination
Using an evidence-based model of measurement-based care tracks success for each individual consumer
Qualifications:
Requires bachelor’s degree Human Services or related field
Case Management Experience
Communication, organization, time management and clinical skills.
Ability to work independently in a community-based setting.
Valid Colorado Driver’s license with a clean driving record.
Bilingual Spanish preferred, but not required
Physical Requirements:
Regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and keyboard.
Specific vision abilities required by this job include close vision requirements due to computer work.
Light to moderate lifting is required.
Regular, predictable attendance is required.
Schedule:
Monday-Thursday 8am-7pm, office based, in person, some travel in the community
Salary Information:
$24.04 to $27.00/hr
Eligible for $250/month Bilingual Stipend
